Endovascular recanalisation of an acute distal AVF thrombosis with EKOS ®

A 61 years old woman on hemodialysis by a left radio-cephalic arteriovenous fistula (AVF) presented a left cephalic vein thrombosis involving a venous tract of about 20 cm of length including two aneurysmal dilatations 7 and 18 cm from the arteriovenous anastomosis (AV) respectively.
Thrombosis extension prevented us from performing mechanical thrombolysis or surgical trombectomy and induced us to use endovascular treatment with ultrasonic (US) energy combined with thrombolitic infusion deep into the clots. No description about this treatment option performed on AVF was reported in literature.
EKOS ® system permitted a rapid resolution of vein thrombosis, restoring the patency of the forearm superficial venous circle without any injury and allowing an immediate use of AVF for dialysis. Moreover, this system allowed a reduction of thrombolitic agent dose, lowering the incidence of bleeding complications.
